Electronic Visit Verification Inventory Survey

 

Background

 

Thank you for agreeing to participate in this important survey about electronic visit verification (EVV). The Minnesota Department of Human Services (DHS) is surveying providers of personal care assistance, home health services, and home and community-based services to learn more about existing use of EVV in Minnesota.

The 21st Century Cures Act directs states to require EVV for personal care services, home health services, and home and community based services that provide support for activities of daily living or instrumental activities of daily living. According to the Act, EVV must verify at minimum seven data elements related to the service: type of service performed, who received the service, date of service, location of service delivery, who provided the service, when the service begins and when the service ends.

 

The 21st Century Cures Act and 2017 Minnesota Session Laws 1st Spec. Sess. Chapter 6, article 1, section 49 require the state to consider existing practices. Your responses are very important in helping DHS better understand the types of service verification systems currently in use in Minnesota.

The survey should take about 10 minutes to complete.

 

Data privacy

 

DHS has retained Management Analysis and Development (MAD) to analyze the survey responses and draft a report based on the findings. MAD is a division within Minnesota Management and Budget that provides neutral, third-party consultation services to public sector entities.

Any private information, such as your name, that you provide is protected under the Minnesota Data Practices Act (Minnesota Statutes §13.64). In addition, your name and any information in your responses that identifies you will be removed before the survey data is sent to DHS.
